This Coaching Sessions policy (Marriage and Relationship Coaching) is entered into by and between the undersigned client ("Client") and NaShauna Haddock, Marriage and Relationship coach. By signing below, Client agrees to abide by the terms and conditions outlined in this Contract.

1. COACHING SESSIONS DETAILS:

a. Session Rate: $100 per coaching session.

b. Frequency: [ ] Weekly [ ] Bi-Weekly 

c. Duration: Each coaching session is approximately 60 minutes.

 

2. PAYMENT DETAILS:

a. Client authorizes the automatic deduction of $100 from the provided payment method before each scheduled coaching session.

b. Payments will be processed [ ] Weekly [ ] Bi-Weekly based on the selected frequency.

 

3. SCHEDULING:

a. Coaching sessions will be scheduled in advance and agreed upon by both parties.

b. Rescheduling or canceling a session must be communicated at least 24 hours before the scheduled time.

 

4. SESSION FORMAT:

a. Coaching sessions will be conducted via [ ] In-person [ ] Phone [ ] Video Call (Zoom, Skype, etc.).

b. Client is responsible for ensuring a quiet and confidential environment during the coaching sessions.

 

5. CONFIDENTIALITY:

a. Coach agrees to keep all information shared by the Client confidential, except in cases where disclosure is required by law.

 

6. COACHING OBJECTIVES:

a. Client and Coach will collaboratively establish coaching objectives and goals.

b. Coach will provide support, guidance, and strategies to help the Client achieve their relationship and personal development objectives.

 

7. PAYMENT METHOD:

a. Client shall provide a valid credit/debit card for automatic payment processing.

b. Any changes to the payment method must be communicated to the Coach in writing at least 7 days before the next scheduled session.

 

8. TERMINATION:

a. Either party may terminate the coaching relationship with written notice.

b. In the event of termination, the Client will be responsible for any outstanding fees for sessions already conducted.

 

9. MISSED SESSIONS:

a. If the Client misses a scheduled session without providing 24-hour notice, the session fee of $100 is still applicable.
